Vision Screenings
Grand County School District complies with state law and Utah Department of Health recommendations regarding routine vision screenings for school aged children. Vision screenings are NOT a substitute for a comprehensive eye exam from by an eye doctor. Parents or teachers may request a vision screening from the nurse if there are vision concerns.
- Tier I: The school nurse will screen all Pre-K, Kindergarten, 1st, 3rd, 5th, 7th and 9th grade students for distance vision.
- Tier II: The tier II may include screening for near vision, convergence, tracking and color vision deficiencies. Students in grade 1st-3rd who do not meet reading benchmarks, SPED referrals and re-evaluations will receive both a Tier I and Tier II screening.
Parents will receive a referral and a follow-up phone call from the school nurse if your child did not pass a screening, or if there are other vision concerns. Children who are uninsured may qualify for a voucher for a free eye exam and glasses. Parents should ensure that there is a free/reduced lunch application on file to qualify for a voucher and consult with the school nurse.
Parents or community members interested in volunteering with vision screenings should contact the school nurse.
Vision Resources
Grand County School District partners with the Moab Free Health Clinic and the Hope Alliance to provide free vision services to our students three times a year. Please contact the Moab Free Health Clinic or the school nurse for referral to these clinics. The next clinic is March 25, 26 and 27, 2022. Register with the MFHC here.
Hearing Screenings
Grand County School District complies with state law and Utah Department of Education recommendations regarding routine hearing screenings for school aged children. Hearing screenings are NOT a substitute for a comprehensive hearing exam from an audiologist. We routinely screen the hearing of Kindergarten, First, Second, Third, Seventh, and Eleventh grade students. If any parent/ guardian does NOT want to have their student's hearing screened, please complete the Opt-Out form provided below.
In addition, any parent/ guardian of a student outside of the grades listed above who is concerned about their child's hearing can request a hearing screening from the GCSD Speech department by sending an email request to [email protected] or calling (435) 719-4774.
El Distrito Escolar del Condado de Grand cumple con la ley estatal y las recomendaciones del Departamento de Educación de Utah con respecto a los exámenes de audición de rutina para niños en edad escolar. Los exámenes de audición NO sustituyen a un examen completo de audición realizado por un audiólogo. Regularmente evaluamos la audición de los estudiantes de jardín de infantes, primero, segundo, tercero, séptimo y undécimo grado. Si algún padre / tutor NO desea que se revise la audición de su estudiante, complete el formulario de exclusión voluntaria que se proporciona a continuación.
Además, cualquier padre / tutor de un estudiante fuera de los grados enumerados anteriormente que esté preocupado por la audición de su hijo puede solicitar una evaluación auditiva al departamento de habla del GCSD enviando una solicitud por correo electrónico a [email protected] o llamando al (435) 719 -4774.
