FEE WAIVER INFORMATION Some fees may be waived upon approval of a Free/Reduced Meals application. Foster children are eligible for free or reduced-price meals regardless of the household income. Eligibility for free meals may also be used to determine if students are eligible for additional benefits in other school related programs. Please note that income for Fee Waiver Applications MUST be verified. Contact the building secretary or the School District Office for further information. |

NEED STUDENT INSURANCE??? Each school year students are injured who do not have adequate medical insurance. The perception that the School District will pay the medical cost of injuries that are sustained at school or in school related activities is erroneous. Typically, schools do not carry insurance to cover student accidents. Student accident insurance is, however, made available to students whose parents want coverage for their children. The insurance policy is a limited one, it is voluntary, and offered as a service to parents from a private insurance carrier.. TO AVOID MISUNDERSTANDINGS --- BE SURE TO READ THE ENTIRE POLICY CAREFULLY! If you are interested, enrollment forms may be obtained during registration, or brought home by your student(s) shortly after school begins. Also, students involved in athletic activities sponsored through the Utah High School Activities Association must have insurance coverage–either through their parent’s family plan or some other arrangement. |

IMMUNIZATION REQUIREMENTS |

UTAH LAW REQUIRES THAT - 1. children between the ages of 6-18 be enrolled in school. 2. children enrolled in kindergarten must be five years of age on or before September 2. 3. all students must meet the immunization requirements listed. 4. students enrolling for the first time in the school district are required to show a certified copy of the student’s birth certificate, or other reliable proof of the student’s identity and age. |

Beginning 1999-2000, Hepatitus B became a requirement for kindergarten entry.
Beginning with 2002-2003 school year, Hepatitis A and Varicella became required for kindergarten entry. Beginning 2006-2007 Hepatitis B, Tetanus/Diphtheria booster (Td), and Varicella were required for 7th grade entry. Effective May 7, 2007, the Tetanus/Diphtheria/Pertussis (Tdap) vaccine is required for students entering the 7th grade. Additionally, all Utah students, kindergarten through grade 12, are now required to have two doses of the Measles, Mumps, Rubella (MMR) vaccine, rather then one dose of the MMR and one additional dose of the measles vaccine. The rule change allows either Tdap or Td vaccine to satisfy the 7th grade requirement and allows previous receipt of two doses of the measles vaccine for the two dose MMR requirement. Immunization Exemptions from School Law? There are three types of exemptions available. Exemption forms must be given to the school to be placed in student’s permanent file. Medical -The child’s licensed physician must complete a Medical Exemption Form. Personal - Parents/guardians who have personal or philosophical beliefs against immunization. Ask health department official for help to complete this form. Religious - Parent/guardian who is a member of religious group whose doctrine opposes immunization. Ask health department official for help to complete this form. |

BUS SAFETY Beginning this fall, District Transportation will no longer be allowed to drop students off at a destination other than his or her home bus stop. This means parent permission notes, or phone calls, requesting a student be dropped at an alternative location (a friend or relative’s house, scouts, etc.) can no longer be accepted. State Law requires State Transportation Funds be “used to reimburse districts for the direct costs of transporting students to and from school.” Due to liability issues, GCSD must comply by transporting students to and from their residence only. Parents whose student(s) have a regular daily child care destination after school, may make special arrangements by calling the District Transportation Office at 259-5430. Please be aware that rules related to riding school buses come from state law and have been added to the GCSD Discipline Policy, also video/audio tapes are made of bus trips to help assist with students' safety. If a problem arises regarding the transportation of students, please call the Transportation Supervisor at 259-5430 during business hours. |
